Communication seems so natural that we rarely stop to think about what it actually requires. We hear a sound, figure out what it means, and respond – all within fractions of a second. But behind this effortless exchange lies a complex set of abilities working together in precise coordination. For children with intellectual disability (ID), one or more of these abilities may be compromised, making the act of communicating a real and daily challenge. Understanding which abilities are affected – and why – is the first step toward meaningful support.

The foundational abilities for communication
Before a child can hold a conversation, share a thought, or even follow a simple instruction, several core abilities need to be in place. According to speech and language health services, the most important skills needed for communication are attention and listening – all other communication skills are built upon these. But that’s just the starting point. Effective communication also requires the ability to:
- Hear – physically receive spoken sounds
- Attend – focus on what is being said
- Screen and select – filter out irrelevant noise and focus on relevant speech
- Understand – process and make meaning from language
- Take turns – participate in the back-and-forth exchange of conversation
- Express – convey thoughts, needs, or feelings to another person
A gap in any one of these areas can create significant communication difficulties. Research published in the International Journal of Environmental Research and Public Health confirms that individuals with intellectual disabilities frequently need support with both understanding and expressing themselves – and that these difficulties directly affect their quality of life. For children with ID, the challenge is rarely just one gap. Often, several of these foundational abilities are affected at once.
The role of hearing in verbal communication
Hearing is the gateway to verbal communication. Without the ability to receive sound, the entire chain of spoken language breaks down. This is why hearing screenings are considered essential when assessing children with communication delays. As Connected Speech Pathology notes, hearing loss can significantly affect a child’s ability to develop speech and language skills – if a child cannot hear sounds clearly, they may struggle to imitate and produce them correctly, leading to delayed communication development.
Children with intellectual disability are more likely than their typically developing peers to have co-occurring hearing impairments. A review published in Child and Adolescent Psychiatric Clinics of North America notes that children with ID are at higher risk for a range of sensory impairments, including vision and hearing difficulties, and that these comorbidities can further increase communication barriers. This is why a thorough audiological assessment should always be part of any communication evaluation for a child with ID.
When hearing is intact but processing isn’t
It’s important to distinguish between hearing and listening. Hearing is physiological – it’s the ear receiving sound waves. Listening involves the brain actively making sense of those sounds. Communication scholars describe it this way: while hearing leads to listening, they are not the same thing. External noises interfere with the physiological aspects of hearing; internal noise – such as limited cognitive processing – interferes with the cognitive process of listening itself.
For children with intellectual disability, even when the ears are physically functional, the brain’s ability to process and interpret incoming language may be slower or less efficient. This means a child might technically hear what is being said without truly understanding it – a distinction that has significant implications for how we communicate with and teach these children.
The screening and selecting ability: filtering what matters
Imagine sitting in a classroom. There’s the hum of an overhead fan, the sound of chairs scraping the floor, voices in the corridor, and your teacher speaking at the front. For most people, the brain automatically filters out the background noise and focuses on the teacher’s voice. This ability – to screen out irrelevant sounds and select the relevant ones – is known as auditory selective attention, and it plays a critical role in communication.
A study published in Psychological Science found that younger children struggle significantly with filtering unpredictable background noise, and that improvements in selective auditory attention develop gradually through childhood, only reaching adult-like levels around ages 9-11. This means that even in typically developing children, this filtering ability takes years to mature fully.
For children with intellectual disability, this maturation may be further delayed or impaired. Research from PubMed on children with language impairments found that children with poor language abilities show notable difficulties with attentional filtering – they struggle to separate relevant speech from competing background noise, which directly undermines their ability to engage with communication in busy, real-world environments like classrooms or playgrounds.
Why noisy environments are especially challenging
A classroom that feels like a perfectly normal environment to a typically developing child can be genuinely overwhelming to a child with ID. Selective listening – the ability to identify and pay attention to important auditory information while filtering out the rest – is a skill that demands cognitive effort. When a child’s brain is already working hard just to process incoming language, the added burden of filtering environmental noise can tip the scales entirely.
This is why speech pathologists and educators working with children with ID consistently recommend reducing background distractions during communication. Minimising noise, using face-to-face positioning, and creating calm, distraction-free environments aren’t just nice-to-haves – they are evidence-based accommodations that address a real neurological limitation.
Challenges in understanding language
Understanding language – known as receptive language – is far more than recognising words. It involves grasping meaning, following instructions, interpreting context, and connecting new information to prior knowledge. According to Vanderbilt University’s Special Education course resources, the majority of individuals with ID are able to develop functional communication skills, but they will often show delays in reaching communication milestones and have specific difficulties across the components of language.
There are two key reasons children with ID may struggle to understand language:
1. Cognitive processing limitations
The brain interprets language by connecting new words and sentences to stored knowledge and past experiences. This interpreting phase can be almost instantaneous when prior knowledge is readily available, or slow and effortful when connections are hard to make. For children with ID, cognitive processing is often slower, meaning they need more time to decode what has been said and attach meaning to it.
This processing delay can easily be mistaken for inattention or non-compliance. A child who doesn’t respond to an instruction isn’t necessarily refusing – they may still be working through what was said. Rushing or repeating instructions rapidly without allowing processing time can make things significantly worse.
2. Limited language experience
Language understanding is built through exposure – the more varied and rich a child’s language environment, the stronger their comprehension tends to be. Children with intellectual disability frequently experience challenges in acquiring and using language, often due to limited interaction opportunities, reduced participation in social communication, and environments that may not adequately stimulate language development.
A study in the Journal of Applied Research in Intellectual Disabilities found that students with ID who attend special schools often have smaller peer networks, reducing opportunities to hear and practise varied language. Since language is learned through interaction, fewer social opportunities can compound comprehension difficulties over time.
Challenges in expressing language
Expressive language – the ability to communicate thoughts, needs, and feelings to others – is the visible output of communication. When this breaks down, the consequences for a child are immediate and often distressing. Research published in the NIH highlights that a major factor underlying challenging behaviours in children with ID stems from communication impairment, which limits their ability to express frustration or explain external factors – and that challenging behaviours occur in as many as 25% of children with ID, compared to 4-9% in typically developing children.
There are two primary reasons why children with ID may struggle to express themselves:
1. Physical limitations of speech production
Speaking involves a precise coordination of breath, vocal cords, the tongue, lips, and palate. For some children with ID – particularly those with associated conditions such as Down syndrome or cerebral palsy – the physical coordination required for clear speech production may be compromised. When the speech organs don’t work in full coordination, verbal expression becomes effortful, imprecise, or impossible without support.

When a home or school environment doesn’t provide sufficient verbal engagement, scaffolding, or responsiveness, expressive language development suffers. A longitudinal study in PLOS ONE also found that individual communication skills and the communicative context of those around a child – including classmates and caregivers – significantly predict changes in behaviour and communication outcomes. In other words, the communication environment is not a passive backdrop; it is an active ingredient in a child’s development.
Why turn-taking is harder than it looks
Turn-taking – the back-and-forth rhythm of conversation – is something most people do automatically. But it requires understanding when to speak and when to listen, reading social cues, holding your own thought while another person speaks, and timing your response. Speech pathologists emphasise that turn-taking is rooted in joint attention – the shared focus between two individuals – and that children with developmental differences often need targeted support to build these early conversational skills.
For children with ID, turn-taking can be disrupted by delayed processing speed, difficulty reading non-verbal cues, or limited experience in conversational exchanges. Communication researchers note that children with intellectual disability may have trouble with social communication – including knowing how to take turns and knowing what to say in different situations. When turn-taking breaks down, conversations stall, misunderstandings increase, and social relationships become harder to form and maintain.
The bigger picture: communication as a chain
What this all points to is that communication is not a single skill – it’s a chain of interdependent abilities. Research on communication support needs in people with ID consistently shows that limitations in communication are directly associated with lower quality of life, reduced social participation, and diminished self-determination. But it also shows that communication skills can be improved with the right support, even in adolescence and adulthood.
For educators, caregivers, and therapists, understanding where in the communication chain a child is struggling – whether it’s hearing, filtering, comprehending, expressing, or taking turns – is essential. Each gap calls for a different kind of support, and no two children with ID will have exactly the same profile.
