Every child’s physical journey begins long before their first steps. From a newborn’s instinct to turn their head, to a six-year-old confidently skipping across a playground, gross motor development is the continuous process by which children gain control over the large muscles of their body. Understanding this progression – and what’s considered typical at each age – equips parents, caregivers, and educators to better support children and recognize when something may need closer attention.

What are gross motor skills?

Gross motor skills involve the large muscles of the arms, legs, and torso working together to produce movement. Walking, running, jumping, climbing, and throwing are all examples. These skills differ from fine motor skills, which involve smaller, more precise movements – like holding a pencil or buttoning a shirt. Gross motor abilities also form the foundation for body awareness, balance, reaction speed, and strength, and they create the physical base from which fine motor skills can develop.

Importantly, gross motor development doesn’t happen in isolation – it is deeply connected to a child’s neurological growth. As the brain matures, it sends more refined signals to the muscles, enabling increasingly complex and coordinated movement.

The sequence of gross motor skill development

One of the most well-established facts in child development is that gross motor skills follow a predictable sequence. Children typically develop control from the head downward and from the center of the body outward – a pattern known as cephalocaudal and proximodistal development. This means head control comes before trunk control, which comes before leg control. As gross motor milestones build upon each other, rolling leads to crawling, crawling leads to pulling to stand, and standing leads to walking.

Understanding developmental norms

A developmental norm is not a fixed point in time – it’s a window of expected achievement. Most milestones have a range of months during which they are typically reached. The WHO Multicentre Growth Reference Study, which tracked 816 children across five countries, found that these windows vary considerably in width. For example, the window for sitting without support spans about 5.4 months, while the window for walking alone spans nearly 9.4 months – reflecting the significant natural variation among healthy children.

This variation is completely normal. Individual differences in temperament, body type, opportunity for movement, and neurological development all influence when a child reaches a given milestone. What matters most is the overall pattern of development, not whether a child achieves a skill on a precise date. That said, consistently missing multiple milestones across an age range – especially in combination – warrants professional evaluation. As Brown University Health notes, these guidelines exist to help identify children who may benefit from early support, not to create unnecessary anxiety for parents.

Milestones from birth to 6 months

The first six months of life are characterized by rapid neurological development and the emergence of foundational movement skills. Every milestone achieved in this period lays the groundwork for all future motor abilities.

Head and neck control (0-3 months)

A newborn’s head control is minimal – the head lags behind the body when pulled to sitting, and the neck muscles are just beginning to strengthen. By two months, most babies can lift their head during tummy time and push up onto their forearms. By three months, they can lift the head further and sustain that position for longer periods. This seemingly simple skill is critical – it builds the neck and shoulder strength that will eventually support crawling and upright posture.

Rolling over (3-5 months)

Rolling is typically one of the first major transitional movements a baby makes. Most infants begin rolling from tummy to back around 3-4 months, and then from back to tummy around 4-5 months. This involves coordinated activation of the core, hips, and shoulder muscles – a significant neuromotor achievement. As each gross motor milestone builds on the last, consistent tummy time directly supports the muscle development needed for rolling.

Sitting with support (4-6 months)

Around 4-5 months, babies can sit with substantial support, such as being propped against cushions or held. By 6 months, many can sit independently for short periods, requiring significant core strength and balance. The WHO data shows the window for sitting without support falls between approximately 3.8 and 9.2 months among healthy children globally – a useful reminder that there is no single “right” age for this milestone.

Milestones from 1 to 3 years

The toddler years are marked by extraordinary physical transformation. Children shift from dependent infants to independently mobile individuals who can explore their environment with growing confidence and control.

Independent walking (12-18 months)

Walking is perhaps the most celebrated gross motor milestone. Although most babies take their first steps close to or just after 12 months, others may begin walking a little earlier or later – and both are considered within the normal range. Early walking is wide-based and wobbly, with arms held out for balance. By 15-18 months, most toddlers walk with greater steadiness and begin to vary their movement, such as stopping and changing direction.

Running and climbing (18 months-2 years)

Once walking is established, toddlers quickly discover running – even if early attempts are more of a fast, stiff-legged trot. By 18 months, most children attempt to run. Climbing onto furniture is also common at this age. Anything a child does to get from one spot to another is locomotion, and toddlers relentlessly practice locomotor skills through free play. Pulling wheeled toys while walking also becomes common, showing the developing ability to coordinate movement with an external object.

Stair climbing and ball play (2-3 years)

By age 2, most children can walk up and down stairs while holding onto a railing or wall, though they typically place both feet on each step rather than alternating. Kicking a stationary ball forward without losing balance also emerges during this period. Children begin to show a greater understanding of their body’s size in relation to the space around them – an important dimension of spatial awareness that supports safe movement.

Advanced skills: 3 to 6 years

The preschool years bring a dramatic refinement of movement quality. Children move from functional but somewhat clumsy locomotion to controlled, coordinated action. Physical play becomes richer, more varied, and socially engaged.

Balance and one-foot standing (3-4 years)

By age 3, most children can balance briefly on one foot – a skill that requires significant postural control and core stability. This milestone underpins many future abilities, including hopping, skipping, and eventually sports-related movements. The American Academy of Pediatrics includes one-foot standing as a key gross motor skill assessed during developmental evaluations in early childhood.

Pedaling a tricycle (3-4 years)

Learning to pedal requires both legs to work in a coordinated, alternating rhythm while the child simultaneously steers and maintains balance. It is a more complex skill than it appears. Most children manage a tricycle between ages 3 and 4, with some progressing to a bicycle with training wheels by age 4-5. This activity also builds leg strength and bilateral coordination – the ability to coordinate both sides of the body together.

Hopping, skipping, and galloping (4-5 years)

Hopping on one foot typically emerges around age 4, followed by galloping and then skipping. By age 6, most children can skip if shown how – a movement that requires a complex combination of muscle strength, balance, coordination, and timing. These locomotor patterns demonstrate that the child’s nervous system has matured to the point of coordinating multiple movement sequences simultaneously.

Throwing and catching (4-6 years)

Ball skills undergo significant development during the preschool years. Four-year-olds typically throw using their whole body – rotating the trunk and stepping forward with the opposite foot. Catching improves from using the whole body to trap a large ball, to catching smaller balls with the hands alone. These skills require eye-hand coordination, timing, and spatial judgment, making them among the more cognitively demanding gross motor activities of early childhood.

When to be concerned: recognizing delays

A single missed milestone is rarely cause for alarm on its own – children develop at different rates, and the normal range is wide. However, consistent signals such as no signs of walking by 18 months, stiff or unusually floppy limbs, persistent difficulty using one side of the body, or the loss of previously acquired skills are worth discussing with a pediatrician promptly. According to the Cleveland Clinic, approximately 1 in 6 children in the United States has at least one developmental delay – many of which are motor-related.

Gross motor delays can stem from a range of causes including low muscle tone, neurological conditions, prematurity, genetic disorders, or simply insufficient opportunity for physical activity. A child with delayed gross motor skills like rolling over, sitting, or walking may be referred to a physical therapist, who can assess strength, balance, and coordination and recommend targeted interventions. The American Academy of Family Physicians advises that clinicians should not take a “wait and see” approach when motor concerns are identified – early referral to intervention services is always preferable to delay.

Research published in PMC by the National Institutes of Health confirms that early intervention takes advantage of the heightened neurological plasticity of the first years of life, maximizing the potential benefit of therapy. Even targeted motor skill training in infants without developmental risk has been shown to support earlier milestone achievement.

Supporting gross motor development at home and in the classroom

Children develop gross motor skills primarily through active, unstructured play. Opportunities to crawl, climb, run, throw, and balance are not extras – they are developmental necessities. Tummy time for infants, access to safe outdoor spaces for toddlers, and varied physical play for preschoolers all contribute directly to motor skill progression. For educators and caregivers working with children who have developmental differences, understanding the expected sequence of milestones helps in setting realistic goals and planning appropriate activities that meet children where they are.
